Prescription Requests

Prescriptions are typically filled at the time of your appointment. If you need a prescription refill that is not associated with an appointment, please call the office at (937) 342-9260 and leave a message on the nursing line or text us at (937) 353-9990 on the nurse line. Please tell us your NAME, DATE OF BIRTH, PHONE NUMBER, PRESCRIPTION NAME and DOSAGE, PHARMACY NAME, and PHARMACY PHONE NUMBER.

Please allow 24-48 business hours for the medication to be called in to your pharmacy. If you have special instructions, please let us know at the time of the refill request (example: you would like a 90-day prescription). In some instances, you may need an appointment before the prescription can be refilled.

Please keep in mind that our office will only call you if your prescription has changed or if we are unable to call the medication in. Please contact your pharmacy 24-48 hours after your request to verify that the prescription has been called in.
