Paperwork:

Fill out the necessary forms prior to your appointment. You should complete these electronically for convenience. If not completed beforehand, please arrive 15 minutes early to complete them at our office.

Identification and Insurance:

Bring a valid form of identification with your current address.

Have your insurance card ready. Without proper identification and insurance, we may need to reschedule your appointment.

Required Documents:

Complete the Information Sheet and History Sheet.

Financial Responsibilities:

Be prepared to pay your co-pay and any deductible that applies. We accept cash, check, VISA, MasterCard, Discover, American Express, or debit card. Payments can also be made electronically or via our online payment portal.

If you are self-paying, please be ready to settle 100% of your account balance.

Additional Requirements:

Provide your social security number for identity verification.

If your photo ID does not show your current address, bring a utility bill or similar correspondence.

Insurance Details:

If you have insurance that wasn't previously reported to us, please inform us before your appointment.

Referral (if required):

Ensure you have obtained a referral from your primary care physician if mandated by your insurance company. We must receive it before your visit to facilitate your consultation.

Appointment Timing:

Arrive on time for your appointment to avoid potential rescheduling.

Appointment Changes:

If you cannot attend your appointment, please notify us promptly.
